Along with the increasingly tight competition, companies are required to always be consistent in improving quality of its products. Improvement product can achieved through minimization or even reduction defects. This study aims minimize defects by providing improvement suggestions based on critical failure modes The Six Sigma approach is adopted reduce occurrence FMEA-FSWARA-FWASPAS FMEA method integrated six sigma approach, especially determine priority and recommended efforts that trigger FSWARA used severity, occurrence, detection weights as mode assessment criteria. Meanwhile, determination results evaluation using FWASPAS. research a case which 5 types were found, namely, skewness, underfilling, leaks, broken lids, boxes. main causes lie human factor machine factor. showed there 3 modes, wrong setting cutter timer operator, frequent change heater temperature, material getting damaged
